Live Well, Be Well serves as the link between you and the State Farm benefits that support your nutrition, fitness, and weight management goals. The following resources can help you learn more:

Coaching and support

  • Noom: This weight-loss platform, delivered through an app on your mobile device, provides holistic guidance to support healthy eating habits, behavior change, and your overall health goals. Noom is available to employees enrolled in a State Farm BCBSIL or Aetna medical plan. Learn more about Noom.

  • Well-Being Coaching through the Employee Assistance Program (EAP): GuidanceResources connects you with caring professionals who can guide you through nutrition, fitness, and wellness goals.
  • Live Well, Be Well sessions: Try a 15-minute A Path to Living Well session that shares simple, actionable tips to help you live healthier. You can also explore Live Well, Be Mindful sessions , featuring chair yoga, gentle stretching, and more to support both your physical and mental well-being.
  • Customized Movement Routines: Start and stay moving with Sword Move, your whole-body solution for a pain-free tomorrow. Sword is available to employees enrolled in a State Farm BCBSIL medical plan.

  • Perks at Work: Enjoy a curated collection of exclusive discounts and free resources that support a healthier lifestyle, including meal options, fitness classes, activity trackers, and wellness content through the Community Online Academy.   • Gym Membership Discounts: If you are enrolled in a BCBSIL medical plan, you can also get discounts on gym memberships, helping you save while you stay fit.

Tobacco cessation

Albeit challenging, quitting tobacco in all forms is one of the best ways to improve your life and health. Our tobacco cessation program can help you succeed.

Employee Assistance Program (GuidanceResources) tobacco cessation program

You and any member of your household who is 18 or older can participate in the tobacco cessation program. You (and they) do not need to be enrolled in a State Farm medical plan to participate.

To get started, call 888-327-7501, or enroll from your GuidanceResources online account (web ID: STATEFARM).

Frequently asked questions

Is this program confidential?

As with all other Employee Assistance Program (EAP) services, participation in the tobacco cessation program is confidential. Basic identifying information is collected to verify eligibility and establish access to services (and to adjust your medical insurance premium upon completion of the program, if applicable). State Farm receives only aggregate data about this program (e.g., how many participants are enrolled).

How much will it cost me to enroll in this tobacco cessation program?

The EAP’s tobacco cessation program is available at no cost to you. If you and your coach determine that you may benefit from nicotine replacement therapy (NRT), you may purchase these products and request reimbursement through the EAP. NRT products are reimbursable up to $150 per calendar year.

What is nicotine replacement therapy?

Nicotine replacement therapy (NRT) comprises gum, patches, or lozenges, and it is used as an aid in addition to counseling to help participants quit tobacco. NRT products are available over the counter and without a prescription. If you choose to use NRT products, you are responsible for purchasing these products, but you are eligible for reimbursement up to $150 per calendar year.  

What about prescription quit aids?

If you need prescription quit aids, you can use your prescription drug benefit. You may need to pay out-of-pocket expenses associated with prescription aids. For more information, call CVS Caremark at 800-388-2058 or visit the website (auto sign-on with SSO | log in).
